Description :

Summary: The Patient Navigator is responsible for assisting Genesis patients in navigating the health and human services system. The Patient Navigator will find out the needs of the client and helps the client to reach her goals and fulfill her needs.
Duties and Responsibilities:

Initial applicant assessment
Obtain the completed application form from the applicant and/ or family (or assist in completion of forms)
Meets with the applicant/family/guardian and gather materials for submission
Clarify the request for services and the needs of the applicant/family
Review and analyze available data; identify barriers and opportunities in applicants’ developmental objectives and plans
Provide information regarding Genesis Community Health and services available
Work with applicants to assess needs and preferences
Confirm any immediate needs and provides referrals for any necessary interventions or services
Schedule and coordinate eligibility meetings with team and applicants
Obtain necessary information to complete application process for programs applied.
Submit complete application packet and release of information to community partners
Develop initial care plan for eligible applicants and transfers case to ongoing providers
Provides information and referrals for applicants who are not eligible for agency services.
Assists patients navigating the numerous clinic processes and policies for scheduling appointments in medical, dental and behavioral clinics.
Interpret and explain clinic policies, regulations, financial obligations, and programs to clients/patients.
Assist patients/clients in preparing necessary forms/paperwork for various services/organizations and agencies.
Act as a liaison between the service providers and advocate for patient.
Regularly review the patients’ progress toward goals.
Coordinates with various service providers to ensure the fulfillment of the patient’s needs.
Maintain all applicant information as confidential in compliance with the Federal Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Knowledgeable about the availability of resources and process to access needed resources
Empower applicants and their families to fully participate in coordination and utilizing the resources available
Participate in staff and other internal meetings and task forces as assigned.
Participate in community meetings and activities
Assist community resources in improving the quality of services to the disabled.
Works collaboratively with interdisciplinary team to implement the Patient Centered Medical Home model
Performs other duties as assigned.

Knowledge, Skills and Abilities:

Must have a warm, outgoing personality.
Must embody the mission and vision of the organization.
Must be able to work efficiently in a fast-paced work environment.
Must have good oral and written communication skills and the ability to work with diverse populations.
Bilingual (English/Spanish or English/Creole) a plus.
Must follow appropriate legal and ethical professional conduct.
Must demonstrate computer knowledge, Microsoft Excel and Word experience is a must. 35 wpm.
Must have a working knowledge of telecommunication systems.
Must have a valid Florida driver’s license.

Education and Experience:

High School Diploma or Higher
1-2 Years experience in the Medical Field
BLS Certification

Job Type: Full-time
Pay: $15.00 – $19.00 per hour
